VA changed how this is rated

VA revised the infectious disease rating schedule effective August 11, 2019. Parasitic infections such as helminthiasis now use the General Rating Formula: 100 percent during active disease, then 0 percent for the infection itself once it resolves, with residuals rated under the body system each affects. Veterans rated before this date may hold a rating under the older criteria.

Situation. Years of documented chronic diarrhea and cramping traced to the old infection.

How the criteria apply. Lasting bowel residuals are rated by analogy under the matching bowel code, for example irritable bowel syndrome at diagnostic code 7319, which now rates 10, 20, or 30 percent.

The worms themselves are treatable; the claim is about what years of untreated infection did to the gut. Chronic bowel trouble, malabsorption, and anemia traced to captivity are the ratable residuals.

What VA looks at

POW status, documentation or credible history of parasitic infection in captivity, and the current chronic residuals in the gut or blood counts.

Bowel pattern, weight and nutrition, anemia history, treatment received.

Evidence that usually matters

Service and POW records, stool studies and treatment records, gastroenterology notes, blood count history.

Common misunderstandings

The presumption is about residuals, not active infection. A normal current stool study does not defeat the claim if the chronic bowel or nutritional damage is documented.

If a VA decision on this came back wrong

Under the Appeals Modernization Act (38 CFR 3.2500), a Higher Level Review or a Board Appeal must be requested within one year of the decision notice. A Supplemental Claim can generally be filed at any time after the decision, but filing any of the three within that one year window is what keeps the claim continuously pursued and protects the original effective date; wait longer and a later award may start from the new filing date instead.
