Anemia

Anemia no longer has a single diagnostic code. VA removed the old hemoglobin based code 7700 in the December 9, 2018 hematologic revision. Each type of anemia now has its own criteria: iron deficiency anemia under diagnostic code 7720, pernicious and B12 deficiency anemia under 7722, folic acid deficiency under 7721, acquired hemolytic anemia under 7723, and aplastic anemia under 7716. Anemia caused by blood loss is rated under the condition causing the blood loss.

Brucellosis
Diagnostic code 6316

Brucellosis is rated under diagnostic code 6316 using the General Rating Formula for Infectious Diseases: 100 percent for active disease confirmed by culture or serologic testing, then 0 percent for the infection itself once it resolves, with any residual disability rated under the body system it affects, including meningitis, liver, spleen, and musculoskeletal conditions. Brucellosis is also on the Gulf War infectious disease presumptive list under 38 CFR 3.317(c).

Campylobacter jejuni Infection
Diagnostic code 6318

Campylobacter jejuni is rated at 100 percent during active infection under 38 CFR 4.88b, DC 6318, with chronic residuals (reactive arthritis, Guillain-Barre Syndrome) rated separately.

Coxiella burnetii (Q fever)
Diagnostic code 6331

Q fever is rated under diagnostic code 6331 using the General Rating Formula for Infectious Diseases: 100 percent for active disease, then 0 percent for the infection itself once it resolves, with any residual disability rated under the body system it affects. Chronic Q fever residuals such as endocarditis, hepatitis, osteomyelitis, post Q fever chronic fatigue syndrome, or vascular infection each carry their own rating.

Hansen's Disease (Leprosy)
Diagnostic code 6302

Hansen's disease is rated under diagnostic code 6302 at 100 percent while active. Afterward, residuals such as peripheral neuropathy, skin changes, and deformities are rated under their own codes. It is a presumptive tropical disease under 38 CFR 3.309(b) for veterans who served where the disease is endemic, and its long incubation means it can surface many years after service.

Helminthiasis Residuals (Intestinal Parasites)
Diagnostic code 6320

Intestinal helminthiasis (worm infection) has no percentage ladder of its own. It falls under diagnostic code 6320 (parasitic diseases otherwise not specified), which uses the General Rating Formula for Infectious Diseases: 100 percent during active disease, then 0 percent for the infection itself, with any lasting residual rated under the body system it affects, most often the bowel codes. Helminthiasis is presumptive for former prisoners of war.

Nontyphoid Salmonella
Diagnostic code 6333

Nontyphoid salmonella infection is rated under diagnostic code 6333 using the General Rating Formula for Infectious Diseases: 100 percent for active disease, then 0 percent for the infection itself once it resolves, with residual disability such as reactive arthritis rated under the body system it affects.

Shigella
Diagnostic code 6325

Shigella is rated at 100 percent during active infection under 38 CFR 4.88b, with chronic residuals (reactive arthritis, hemolytic uremic syndrome, persistent GI dysfunction) rated separately.

Syphilis Residuals
Diagnostic code 6310

Syphilis is rated under diagnostic code 6310, which has no percentage table. It routes each residual to the body system it affects: syphilitic heart disease under diagnostic code 7004, cerebrospinal syphilis under 8013, meningovascular syphilis under 8014, tabes dorsalis under 8015, and dementia associated with neurosyphilis under 9301. A cured or latent infection with no residuals rates 0 percent, which still establishes service connection.

Tropical Disease Residuals (38 CFR 3.309(b))
Diagnostic code 6300s

38 CFR 3.309(b) makes a list of tropical diseases presumptive when they appear within a year of leaving service in an endemic area, or later when the incubation period explains the delay: blackwater fever, cholera, dracontiasis, dysentery, filariasis, leishmaniasis, loiasis, malaria, onchocerciasis, oroya fever, pinta, plague, schistosomiasis, yaws, and yellow fever. Most rate 100 percent while active, then on the residuals each leaves behind.

Visceral Leishmaniasis
Diagnostic code 6320

Visceral leishmaniasis is rated at 100 percent during active disease under 38 CFR 4.88b, DC 6320, with chronic residuals (hepatic, splenic, hematologic) rated separately.

West Nile Virus
Diagnostic code 6319

West Nile virus infection is rated at 100 percent during active infection under 38 CFR 4.88b, with chronic residuals (encephalitis, neuromuscular weakness, persistent cognitive deficits) rated under the affected body system.
