Diagnostic code 7718
By the editorial deskUpdated Jun 13, 2026Sources verified Jun 13, 2026
Presumptive under: PACT Act
Quick summary
Primary myelofibrosis is rated under diagnostic code 7718: 100 percent for continuous myelosuppressive therapy or for 6 months after admission for stem cell transplant, chemotherapy, or interferon; 70 percent when therapy is needed to keep platelets under 500,000; 30 percent when therapy holds platelets in the 200,000 to 400,000 range or white cells at 4,000 to 10,000; and 0 percent when asymptomatic. VA added myelofibrosis to the burn pit presumptive list in 2025.
Key requirements
  • Service connection requires a current diagnosis, an in-service event or exposure, and a medical link (nexus) between the two.
  • VA assigns ratings of 0% to 100% under diagnostic code 7718, using the criteria in the rating table below.
  • The percentage assigned turns on Treatment and blood counts.
  • Presumptive service connection may apply under the PACT Act for veterans with qualifying service.

Asymptomatic

Illustrates 0%

Situation. Primary myelofibrosis is present but asymptomatic.

How the criteria apply. Asymptomatic myelofibrosis rates 0 percent under DC 7718.

Needs ongoing myelosuppressive therapy

Illustrates 30%

Situation. The veteran needs continuous or intermittent myelosuppressive therapy to keep blood counts in range.

How the criteria apply. Requiring myelosuppressive therapy, chemotherapy, or interferon to maintain platelets of 200,000 to 400,000 (or WBC 4,000 to 10,000) rates 30 percent under DC 7718.

The tiers follow how hard the medication has to work to keep your blood counts in range. The lab numbers in the criteria are literal, so bring the actual count history.

What VA looks at

Bone marrow biopsy confirming myelofibrosis, the medication regimen, platelet and white cell count history, and qualifying service for the presumption.

Common exam and DBQ topics

Fatigue, spleen enlargement, medication regimen, count stability, transplant planning.

Evidence that usually matters

Bone marrow biopsy reports, hematology notes, complete blood count history, treatment records, service records.

Common misunderstandings

Leukemic transformation moves the rating to diagnostic code 7703 at 100 percent. Complications such as gout or thrombosis are rated separately on top.

If a VA decision on this came back wrong

You have three review lanes, pick the one that fits the situation.

Under the Appeals Modernization Act (38 CFR 3.2500), a Higher Level Review or a Board Appeal must be requested within one year of the decision notice. A Supplemental Claim can generally be filed at any time after the decision, but filing any of the three within that one year window is what keeps the claim continuously pursued and protects the original effective date; wait longer and a later award may start from the new filing date instead.

State benefits stack with VA disability

Your rating may also qualify you for state-level benefits.

VA compensation is federal, but every state layers its own benefits on top. Many states reduce or waive property tax for disabled veterans, and many offer license, tuition, or vehicle benefits at various rating levels. The thresholds, dollar amounts, and eligibility rules vary widely from state to state, so the reliable answer is always your own state's guide.
