Nerve ratings follow which nerve, which side, and how badly it functions: mild, moderate, or severe. These are commonly missed add ons to spine and diabetes claims.

  • EMG or nerve conduction study results, if any
  • The primary condition driving it (spine condition, diabetes) and its records
  • Which limb and which side: symptoms are rated per extremity
  • Numbness, tingling, burning, or weakness, with specific examples
  • Dropping objects, tripping, or balance changes
  • How far symptoms radiate (where they start and stop)
  • Sleep disruption from limb symptoms
  • Treatment: medication (gabapentin and similar), injections, therapy
  • Whether the symptoms were raised at your spine or diabetes C&P exam
